The EOP&S Eligibility Assessment Form will be available in the EOP&S office beginning at 9 a.m. on Monday, June 14, or you may print the form by clicking the link below. Incomplete forms will not be accepted. Items #1 and #3 must be completed prior to turning in the form to the EOP&S office. Extended Opportunity Programs and Services (EOP&S) was established to help community college students from economically and educationally disadvantaged backgrounds succeed in either transferring to a four-year institution or finding career employment in their field of training.
